Unable to demote Domain Controller


i'm trying demote server 2008 r2 domain controller (call dc4) in order rebuild server 2012 r2.  i've ran through steps here http://technet.microsoft.com/en-us/library/cc816644%28v=ws.10%29.aspx step 6.  commands command prompt show fsmo roles on domain controller (dc2).  when run dcpromo , try demote it fails.

the event log on dc4 shows error "the operations master roles held directory server not transfer"  "5005 directory service missing mandatory configuration information" , warning

"fsmo role cn=infrastructure

fsmo server dn:  cn=dom"

there server named dom years ago on network long gone.  see no reference in active directory , again, command line shows infrastructure role running on dc2.   i've transfered infrastructure role dc , dc2, still unable demote dc4.

any ideas how can demote dc4 and/or remove server dom wherever hiding in active directory?
